Emergency Contact Forms and Pledge Sheets

All, last night we sent home two of the three forms we talked about at the Parent/Guardian’s Meeting. Again, I realize much of the info is what is already contained on physicals and other info sheets you’ve already filled out, but we need this one consolidated form for the school.We’re trying to get these back by tomorrow so we don’t have to worry about them before the game on Tuesday.

We need to have these forms completed and turned in prior to Tuesday in order for your athlete to participate in the game. That is why we’re asking for them by Friday, with Monday at the absolute latest. We appreciate your help in getting these filled out, back to your player so they can turn in to us.

For the Pledge Form as we discussed in the meeting, you can go on this website to the Support & Resources block and click on ‘Resources, Documents and Required Forms’. Once open, you will see “Athletic-Handbook – English” and “BMS-Student-Handbook”. Beneath each link is what pages to read in support of information on the pledge form. If you have any issues, please contact us. Almost all of the points in the two documents were covered in our meeting, so this should be mostly a review.

We will deal with the Athletic Fee forms next week and over the next couple of weeks to get that closed out. Full details on this form, such as how to pay, who is exempt, etc. will be posted here next week.

Berkeley Bulldog Football Mantra
Playing football for Berkeley Middle School is a privilege, and as such the opportunity to be a member of the team is in the hands of each and every player. You must bring the combination of will, skill and attitude to the field every day. Hard work, disciplined actions and a commitment to the program will result in guaranteed success. In the end, we will enjoy a sense of pride in our team, each other, our community and our entire school. The fall 2017 season is a another chance to build upon past season's successes that Bulldog football players showed both on and off the field.
