Remember, if you do not have a physical in either your possession or already turned in to the coaching staff, your athlete will need a physical for this sports year to participate in football. The 2017-2018 sports year for physical purposes started on 1 May 2017. So, physicals must be dated on or after 1 May 2017 in order to be valid for this season (and can be used for the rest of the year for other sports as well, i.e. basketball and baseball, etc.).
There are several options to obtain physicals in the local area. Most often, physicals are obtained through coordination with family doctors utilizing personal insurance coverage. Another option is local pharmacies, such as CVS. The fee for these ranges in amount, starting from around $50.00. A third option is to receive a physical at one of the local high schools on pre-arranged dates. The cost for these is $20.00 per student/athlete (cash or check).
On 25 July 2017 starting at 6:00 pm, physicals will be offered at Jamestown High School.
On 26 July 2017 starting at 3:30 pm, physicals will be offered at Lafayette High School.
Regardless of the means by which you obtain a physical, the form must be completely filled out and signed (athlete, parent/guardian and doctor) in order to be valid. This includes background information on family/athlete, medical history, insurance information and of course, the doctor’s clearance and signature. Remember the 2016 version of the VHSL approved physical is on this website in the “Support and Resources” block. This is the only form that will be accepted by the Berkeley Middle School Athletic Department. Be sure to have this form completely filled out before arriving at your athlete’s physical appointment.
